Humana in Monroe, NC is seeking an Onsite Care Coordination Nurse to support newly admitted members in acute care settings. You will perform face‑to‑face assessments, lead care coordination from hospital to home, identify gaps in care, and collaborate with providers to ensure safe, timely transitions.

The role features a hybrid work model, Monday–Friday 8:00 AM–5:00 PM, 40 hours weekly, with a pay range of $71,100–$97,800 per year.

Qualifications

Current, unrestricted RN license in North Carolina.
Must reside within 30–60 minute drive to Atrium Health Union facility.
Minimum 3 years acute care nursing experience.
Experience with discharge planning or care coordination.
Experience with EMR and Microsoft Office; valid driver's license.

Responsibilities

Conduct face-to-face assessments for newly admitted members at acute care facilities.
Support discharge planning from admission through post-discharge.
Identify gaps in care and develop strategies to address them.
Collaborate with providers, case managers, and care teams for coordinated care.
Educate on diagnoses, medications, and preventive measures to reduce readmissions.
Complete documentation accurately and timely; adhere to infection control and privacy standards.
