Job summary

Humana is seeking an Onsite Care Coordination Nurse in Kentucky to support members in acute care settings. You will conduct face-to-face assessments, lead discharge planning, and collaborate with providers to ensure safe transitions from hospital to home.

The role offers a hybrid work model with in-person engagement at assigned facilities and remote coordination/documentation. Qualified RNs with 3+ years acute care experience and knowledge of care coordination are encouraged to apply.

Qualifications

• Current, unrestricted RN license in Kentucky.
• Within 30–60 minutes driving distance to Baptist Health Deaconess-900 Hospital Drive, Madisonville, KY.
• Minimum of 3 years recent acute care nursing experience (medical-surgical, critical care, heart/lung, skilled nursing, or rehabilitation).
• Experience with discharge planning or care coordination in hospital or health plan settings.
• Experience in Microsoft Office (Word, Outlook, Excel) and electronic medical records.
• Valid driver’s license, dependable automobile, and proof of personal vehicle liability insurance.

Responsibilities

• Conduct face-to-face assessments of newly admitted members in acute care facilities to evaluate needs and refer to Humana programs as appropriate.
• Educate members and families on discharge planning from admission through post-discharge, including follow-up visits with PCPs.
• Identify gaps in care and barriers to discharge; develop strategies to address these issues.
• Collaborate with providers, facility case managers, and other health professionals to ensure timely, coordinated care and discharge planning.
• Provide education on diagnoses, medications, and preventive measures to promote wellness and reduce readmission.
• Complete all required documentation accurately and promptly.
• Maintain infection control, safety, and patient privacy standards.
