Job summary

Humana, Inc. in Hazel Crest, IL seeks an Onsite Care Coordination Nurse to assess newly admitted members and lead discharge planning in acute care settings, ensuring safe transitions from hospital to home in a hybrid work model.

You will collaborate with physicians, facility case managers, and care teams to identify gaps in care, educate members and families, document promptly, and promote ongoing wellness while reducing readmissions.

Qualifications

• Current, unrestricted RN license in Illinois.
• Located within 30–60 minute drive to Advocate South Suburban Hospital.
• Minimum of 3 years acute care nursing experience.
• Experience with discharge planning or care coordination in hospital or health plan settings.
• Proficient in MS Office and EMR systems; valid driver’s license and private vehicle.

Responsibilities

• Conduct comprehensive face-to-face assessments of newly admitted members.
• Lead discharge planning from admission through post-discharge and educate families.
• Identify gaps in care and barriers to discharge; collaborate with care teams.
• Provide education on diagnoses, medications, and preventive measures.
• Document accurately and maintain infection control and privacy standards.
