Job summary

Adobe Population Health (APH) seeks a Part-Time Nurse Practitioner for Telehealth & In-Home Support in the Phoenix area. The hybrid role involves delivering virtual care and in-home visits, with a focus on respiratory and population health needs.

The position requires an MSN with FNP specialization, AZ NP license, CPR/BLS, and 1+ year NP experience. Travel across Maricopa County is expected, with flexible scheduling and benefits.

Qualifications

• One year+ NP experience preferred in primary care/population health/home health.
• Background in care of elderly or chronically ill patients preferred.
• Strong clinical assessment, diagnostic and treatment-planning skills.

Responsibilities

• Conduct in-home assessments including physical, psychosocial, environmental, and safety evaluations.
• Perform telehealth visits to assess, monitor, and manage acute and chronic conditions with a focus on respiratory and population health needs.
• Develop patient-centered care plans to address identified health risks and support outcomes.
