Adobe Population Health is seeking a Nurse Practitioner for Telehealth & In-Home Support in a hybrid role. You will deliver direct clinical care via telehealth and in-home visits, travel across Flagstaff, Coconino County, and surrounding areas, and coordinate care with multidisciplinary teams.

The ideal candidate holds an MSN with FNP specialization, is licensed in Arizona, and is proficient with EHRs and telehealth tools. Willingness to travel and strong patient education skills are essential.

Qualifications

• Master’s degree in nursing (MSN) required.
• Family Nurse Practitioner (FNP) specialization required.
• Arizona NP license or eligibility required.
• Current CPR and BLS certification required.
• Experience in primary care or population health preferred.
• Willingness to travel 75-90% within regions.

Responsabilités

• Conduct in-home assessments and telehealth visits for patients.
• Develop patient-centered care plans to improve outcomes.
• Coordinate care with physicians, nurses and case managers.
• Educate patients on disease management and preventive care.
• Maintain accurate documentation in EHR and population health platforms.
