Qualifications

• 5+ years of professional experience in social work, case management, or a related social-services field.
• Strong interpersonal skills and the ability to build trust with individuals from diverse backgrounds.
• Genuine empathy and strong social awareness with the ability to respond to sensitive situations.
• Excellent written and verbal communication skills, including documenting cases clearly.
• Exceptional attention to detail and strong organizational skills.
• Strong assessment, critical-thinking, and problem-solving abilities.
• Ability to manage multiple cases and follow-up activities with accuracy.
• Resilience and composure in emotionally challenging situations.

Responsibilities

• Meet with applicants to understand circumstances, needs, challenges, and goals, creating a supportive environment.
• Review cases and contribute recommendations during case and grant review discussions.
• Develop meaningful relationships with grant recipients and serve as a trusted resource.
• Establish individualized plans with defined goals and measurable outcomes.
• Maintain regular contact with recipients to monitor progress and identify changing needs.
• Evaluate case progress and communicate updates and recommendations to the team.
• Maintain thorough case records and documentation.
• Prepare clear, professional case summaries and notes.
• Develop a network of community resources and services to share with applicants and recipients.
• Build relationships with nonprofits, social workers, and referral partners to expand support.
• Identify communities that could benefit from programs and contribute to outreach.
• Handle sensitive information with discretion, empathy, and sound judgment.
