Job summary

Reserve Care in Cleveland is seeking a Medical Scheduler & Coordinator of Services-Hybrid to organize and manage care for the Western Reserve Navigator program in Cleveland. You will act as the primary contact for patients, physicians and referral sources, triage calls, and coordinate admissions and discharges to ensure seamless coverage of health care needs.

• Minimum three years’ experience in a healthcare environment.
• Proficient in Microsoft Word, Excel, Power Point, Access and Outlook.
• Strong communication skills and ability to multitask in a fast-paced setting.

Responsibilities

• Coordinate services for the Western Reserve Navigator program to ensure seamless coverage.
• Screen calls to triage medical and psychosocial needs and refer as needed.
• Coordinate referrals, admissions and discharges within the agency and with partners.
• Maintain medical files, document in EMR and prepare reports for leadership.
• Educate providers and referral sources on program components and available support.
