Job summary

Reserve Care is seeking a capable coordinator to manage the Western Reserve Navigator program, ensuring seamless coverage of patient healthcare needs. You will interact with patients, physicians and referrals, triage calls, and oversee referrals and admissions while supporting staff efficiency and documentation.

Ideal candidates have healthcare experience, strong communication, and the ability to multitask in a dynamic environment. Healthcare environment familiarity is preferred.

Qualifications

• High school diploma required; associate degree preferred.
• Minimum three years’ experience, preferably in healthcare.
• Proficient in Microsoft Word, Excel, PowerPoint, Access and Outlook.
• Ability to use critical thinking when making decisions.
• Must demonstrate service excellence skills according to agency standards.
• Strong communication skills.
• Ability to multitask in a fast-paced environment.
• Must be self-motivated and work well independently and with others.

Responsibilities

• Serve as point of contact for Western Reserve Navigator to coordinate services with patients, physicians, and referral sources.
• Screen calls to determine medical and psychosocial needs and triage accordingly.
• Facilitate referrals to hospice level of care when necessary.
• Coordinate care within the agency and with external partners.
• Educate allied health providers and referral sources on program components and available support services.
• Oversee operational workflow by scheduling all staff to support efficiency and patient satisfaction.
• Monitor and maintain referral practice and procedures for timely responses.
• Support management to direct operations and assist in implementing procedures.
• Obtain, compile and maintain medical files; ensure daily EMR documentation.
• Generate and submit weekly, monthly, quarterly and annual reports.
• Coordinate admissions and discharges.
• Investigate and manage service failures and provide recovery actions.
• Verify payor source or refer to Patient Financial Advocate to optimize reimbursement.
• Manage and maintain medical equipment for clinical staff.
• Communicate after-hours activity to enhance continuity of care.
• Prepare materials for team meetings and staff communications.
• Assist with various administrative duties.
