Responsibilities

• Coordinate services for participants through Medicaid Waiver programs and MA State Plan.
• Develop and update individualized service plans on a quarterly basis or as needed.
• Monitor participant outcomes and ensure services align with goals and funding.
• Enter documentation accurately within established timelines and maintain compliance.
• Collaborate with providers, agencies, and interdisciplinary teams to support care.
