AdviniaCare Newburyport is seeking a skilled MDS Nurse (hybrid) to coordinate and complete MDS assessments and collaborate with the care team to develop individualized plans. This role supports Medicare/Medicaid documentation and reimbursement efforts.

The position offers 1-2 days of remote work and requires an RN or LPN license in Massachusetts, with MDS experience in long-term care and strong communication.

Qualifications

• MDS experience required and long-term care familiarity.
• Strong communication skills and collaborative mindset are essential.
• BLS certification is required.

Responsibilities

• Coordinate and complete accurate MDS assessments per state and federal guidelines.
• Work with interdisciplinary teams to develop individualized care plans.
• Manage Medicaid documentation and assist with Medicare admissions.
• Support optimal reimbursement through compliant assessments.
• Keep resident records organized, timely, and audit-ready.
