Job summary

Heritage Health Network (HHN) is seeking a Licensed Vocational Nurse (LVN) to join our Enhanced Care Management team, supporting members within the IEHP Pod. The LVN will collaborate with RNs, Lead Care Managers, Community Health Workers, and the Director of Clinical Operations to deliver coordinated, whole-person care in a member-facing role.

Qualifications

• California LVN license in good standing.
• Completion of an accredited LVN program.
• 2+ years of nursing experience, preferably in care management, ambulatory care, ECM, or managed care.
• Strong clinical assessment, care planning and care coordination skills.
• Knowledge of Medi-Cal or managed care practices.
• Bilingual English/Spanish is a plus.

Responsibilities

• Provide clinical support and care coordination for an ECM caseload.
• Support person-centered care plans and address barriers to care.
• Conduct member visits in the community or home and accompany to appointments.
• Complete risk assessments and update care plans with RNs and Lead Care Managers.
• Use motivational interviewing to engage members and encourage adherence.
• Coordinate with hospitals and providers on discharge planning and transitional care.
• Review documentation and ensure accuracy and completeness.
• Participate in daily pod huddles and multidisciplinary reviews.
• Help members access healthcare services and community resources.
• Maintain timely documentation in ECW and ensure regulatory/billing readiness.
• Escalate cases outside LVN scope to the RN or Director of Clinical Operations.
