Job summary

UMMA Health in Huntington Park, CA seeks a Licensed Vocational Nurse (ICP) to coordinate and deliver clinical services for enrolled clients. This role operates at 0.50 FTE for case management and outreach, under the Integrated Care Program Manager.

You will evaluate treatment plans, monitor patient well‑being, and prepare progress reports while educating clients about care options in the community.

Qualifications

• Graduate from an approved nursing program with degree or equivalent
• California LVN license or interim permit required
• Current CPR/BLS Certification required
• Knowledge of Public Health Nursing and Clinic Nursing advantageous
• Aggressive in developing patient care programs within policies and funding requirements
• Sensitivity to needs of patients in a community health center setting

Responsibilities

• Evaluate patients' treatment plans and monitor well‑being
• Create case progress reports and inform patients about care options
• Coordinate medical services for enrolled clients and outreach activities
• Provide health education on chronic conditions to clients and in the community
• Participate in weekly case conferences and provide medical updates
• Identify and submit needed medical supplies to support clinical and outreach work
• Prepare monthly reports on case management services and outreach activities
• Input medical outcome data into the EHR system
