Job summary

Heritage Health Network (HHN) is seeking a Licensed Vocational Nurse (LVN) to join our Enhanced Care Management team serving Medi-Cal members in the IEHP Pod. You will perform clinical assessments, support care plans, and coordinate services with RNs, care managers, and community health workers in a member-facing role.

Based in Riverside, CA and field-hybrid, you will conduct home or community visits, document in ECW, and help reduce avoidable ER visits.

Qualifications

• Active California LVN license in good standing.
• Completion of an accredited LVN program.
• 2+ years of nursing experience in care management, ambulatory care, ECM, managed care, or related setting.

Responsibilities

• Provide clinical support and care coordination for an assigned caseload of ECM members.
• Support individualized, person-centered care plans and address barriers to care.
• Conduct member visits in the community or at home and accompany members to medical appointments.
• Complete risk assessments and collaborate with RNs and Lead Care Managers to develop/update care plans.
• Use motivational interviewing to engage members and support adherence and behavior change.
• Coordinate with hospitals and providers on discharge planning and transitional care.
• Review clinical documentation and care plan notes for accuracy and completeness.
• Participate in daily pod huddles, case conferences, and multidisciplinary reviews.
• Help members access appropriate healthcare services, resources, and community support.
• Maintain accurate, timely, compliant documentation in ECW.
• Ensure documentation supports health plan, regulatory, billing, and payment requirements.
• Escalate cases outside LVN scope to RN or Director of Clinical Operations.
