Job summary

Qualifications

• Active California LVN license; multi-state eligibility including NV, AZ, HI.
• Minimum 1 year of clinical experience (acute care, ambulatory, case management).
• Experience in a managed care environment (health plans, IPA or MSO).
• Proficiency with Microsoft Office and EMR navigation.

Responsibilities

• Comply with company policies and procedures.
• Assess medical, social, and behavioral needs of members.
• Complete member assessments within required timeframes and identify those who may benefit from case management.
• Ensure timely, accurate documentation in all systems.
• Develop and implement an individualized case management plan with stakeholders.
• Collaborate with hospitalists and discharge planners for safe discharge plans.
• Review medical records to determine medical necessity.
• Conduct medication reconciliation post-discharge.
• Review medications for accuracy and interactions; promote adherence.
• Close open quality measure gaps.
• Attend and participate in meetings and IDT, as assigned.
