Granite Peak Home Health seeks a licensed practical nurse to join our home-health team in Montana. The role focuses on implementing care plans, delivering nursing services, and educating patients and families in a home setting.

You will assess patient needs, collaborate with RNs and physicians, maintain accurate records, and participate in conferences. A flexible schedule with on-call duties is required, supporting high-quality patient care.

Qualifications

• Graduate of a state-approved school of practical nursing and licensed in the practicing state.
• Minimum of one year nursing experience preferred.
• Acceptance of agency philosophy and goals; able to exercise initiative.
• Ability to work independently and with the care team.

Responsibilities

• Adhere to policies and procedures and implement the care plan for each patient.
• Provide nursing services, treatments, and diagnostic procedures as assigned.
• Initiate preventive and rehabilitative nursing procedures when appropriate.
• Observe and report changes in patient condition to physicians and RNs.
• Educate patient and family on nursing care needs and self-care techniques.
• Evaluate nursing service effectiveness with RNs and document findings.
• Maintain complete patient records and participate in chart audits.
• Attend staff meetings, care conferences, and in-services.
• Take on-call duties, nights, weekends, and holidays as assigned.
• Prepare equipment and materials for treatments and ensure aseptic technique.
