Job summary

Granite Peak Home Health in Billings, MT is seeking a dedicated Licensed Practical Nurse (LPN) to join our growing home health team. You will implement care plans, provide nursing services, and collaborate with physicians and RNs to support patients at home.

Applicants should have a state-approved LPN program graduate and an active license, with at least two years of nursing experience, ideally in home health. We offer a flexible schedule, competitive pay, and benefits.

Qualifications

Graduate of state-approved practical nursing program and licensed in the state(s) where practicing.
Minimum of two years' nursing experience; home health preferred.
Ability to exercise initiative and independent judgment.

Responsibilities

Implements the nursing care plan for each patient.
Provides nursing services, treatments, and diagnostic and preventive procedures as assigned.
Initiates preventive and rehabilitative nursing procedures as appropriate.
Observes signs and symptoms and reports to the physician and RN changes in patient condition.
Teaches and counsels patient and family regarding nursing care needs.
Maintains accurate and complete records of observations, treatments, and care of patients.
Participates in medical record audits and attends staff meetings as scheduled.
Takes on-call duties, nights, weekends, and holidays as assigned.
