Granite Peak Home Health is seeking a licensed practical nurse to join our growing team in a hybrid home health setting. The role requires implementing care plans, administering nursing services, and educating patients and families.

The ideal candidate has an active LPN license and at least one year of nursing experience, with the ability to work nights, weekends, and holidays as needed. Strong independent judgment and documentation skills are essential.

Qualifications

• Licensed Practical Nurse with state license.
• Minimum 1 year nursing experience, preferred.
• Must accept agency philosophy and goals.
• Ability to exercise initiative and independent judgment.

Responsibilities

• Understands and adheres to established policies and procedures.
• Implements the nursing care plan for each patient.
• Provides nursing services, treatments, and diagnostic procedures as assigned.
• Initiates preventive and rehabilitative nursing procedures as appropriate.
• Observe signs and report to the physician and RN changes in patient condition.
• Educates patient and family regarding nursing care needs and problems.
