Granite Peak Home Health in Kalispell, MT is hiring a Licensed Practical Nurse (LPN) with home health experience to implement and monitor individualized patient care plans. The role requires adherence to policies, accurate documentation, and collaboration with physicians and RNs.

Responsibilities include providing nursing services, teaching patients and families, and participating in care conferences. On-call duties, nights, weekends, and holidays may be assigned as part of the schedule.

Qualifications

• Graduate of a state-approved school of practical nursing and licensed in the state(s) where practicing.
• A minimum of 1 year nursing experience preferred.
• Ability to exercise initiative and independent judgment.

Responsibilities

• Adheres to established policies and procedures.
• Implements the nursing care plan for each patient.
• Provides nursing services, treatments, and diagnostic/preventive procedures as assigned.
• Initiates preventive and rehabilitative nursing procedures as appropriate.
• Observes and reports changes in patient condition to physician/RN.
• T conducts patient and family education on care needs.
• Maintains accurate patient records and participates in chart reviews.
• Attends staff meetings and care conferences; participates in on-call rotations.
• Prepares equipment and materials for treatments; ensures aseptic technique.
• Assists physician and RN with specialized procedures.
