Job summary
Granite Peak Home Health in Montana seeks a dedicated LPN/LVN to join our home-based care team. You will implement care plans, provide nursing services, and support patients and families in their homes.
The role requires a state-approved nursing program graduate with an active license, at least one year of nursing experience, and the ability to work independently. On-call duties, a hybrid schedule, and collaborative care with RNs are expected.
Qualifications
- Graduate of state-approved practical nursing program and licensed to practice.
- Minimum of one year nursing experience preferred.
- Acceptance of agency philosophy and goals.
- Ability to exercise initiative and independent judgment.
Responsibilities
- Implements the nursing care plan for each patient.
- Provides nursing services, treatments, and diagnostic procedures.
- Observes signs/symptoms and reports changes to physician and RN.
- Teaches patient and family about nursing care needs.
- Maintains accurate patient records and participates in audits.
- Attends staff meetings and care conferences.
- On-call duty, nights, weekends, and holidays as assigned.
- Assists physician and RN with specialized procedures.
