Job summary

The Urgent Community Response team at Dorset HealthCare University NHS Foundation Trust is seeking an experienced Occupational Therapist/Physiotherapist for 23 hours per week on a fixed-term basis. You will deliver urgent home-based assessments and short-term interventions to patients in their own homes, including care homes, in partnership with multidisciplinary colleagues.

• Role requires extensive clinical skills to assess and intervene in patients' homes.
• Excellent communication with multidisciplinary teams and carers.
• Ability to work in high-pressure urgent care settings.

• Provide a 2-hour response to health crises in patients' homes.
• Assess, plan and deliver short-term interventions and discharge to appropriate care.
• Collaborate with 111, 999, community teams and care homes.
