The role emphasizes collaborative care with a multidisciplinary team and crisis intervention when needed. This full-time position offers a hybrid schedule with three days on site and two days telehealth, flexible hours, and after-hours support.

Qualifications

• Fully licensed in Illinois as LCSW, LCPC or LMFT.
• Experience conducting comprehensive assessments and developing treatment plans.
• Proficiency in using an EMR system and maintaining updated records.
• Understanding of mental health disorders and treatment modalities.
• Willingness to see a broad spectrum of mental health conditions and provide crisis intervention when needed.

Responsibilities

• Diagnose and treat mental health disorders and provide psychotherapy.
• Develop individualized treatment plans and track progress.
• Collaborate with an integrated care team for coordinated care.
• Provide crisis intervention when needed and ensure safety.
• Maintain accurate, HIPAA-compliant documentation and records.
