Job summary

PruittHealth Corporation seeks a Lead Nurse Practitioner to drive the NP program for PHP, an I-SNP Medicare Advantage plan. You will establish protocols, guide NP leadership, and collaborate with physicians, case managers, social workers, and SNF staff to meet model-of-care goals.

The role is primarily remote with limited daytime travel to the corporate office and SNFs. Ideal candidates have long-term post-acute care experience and strong leadership in care planning, audits, and CMS

Qualifications

• Graduate of an accredited Adult, Geriatric or Family NP program.
• Current, unrestricted RN and NP licensure in state of practice.
• CPR certification and ongoing credential maintenance.

Responsibilities

• Provide onboarding and training for Plan Provider NPs.
• Deliver clinical and personnel development feedback.
• Participate in QAPI activities and quality initiatives.
• Articulate plan benefits and policies to providers and members.
• Coordinate care across care settings and ensure timely ICP communication.
