Job summary

86Borders, LLC. is seeking an experienced Nurse Practitioner to lead and oversee the clinical outreach program for complex patients.

You will manage a team of nurses, create and review patient care plans, and handle challenging patient scenarios while reporting to the Director of Care Coordination with a dotted line to the Chief Medical Officer. The role emphasizes leadership, remote work, and collaboration across healthcare professionals to optimize outcomes.

Qualifications

• Active NP license in TX; ability to obtain others
• Board certification by ANCC/AANPCB or equivalent
• Graduate of ACEN or CCNE-accredited nursing program
• 5+ years leadership experience
• 2+ years remote work experience
• Dedicated home office for remote work
• Experience with Veterans/Medicaid/Dual Eligible patients
• Strong interpersonal and written communication skills
• Experience documenting case notes in care management/EHR
• Experience with motivational interviewing

Responsibilities

• Build care plans for complex patients
• Oversee nurse outreach to complex patients
• Handle clinical escalations
• Collaborate with nurses, care coordinators, specialists, pharmacists, and other healthcare professionals
• Partner with Chief Medical Officer and clinical staff to enhance operations
• Partner with Director of Care Coordination on onboarding and team development
