Job summary

Qualifications

• MSW degree from an accredited program.
• Current, unrestricted NC licensed LCSW.
• Experience with collaborative care models is preferred.

Responsibilities

• Support care for an assigned caseload of 75-80 patients.
• Assess, plan, implement, monitor psychosocial health services.
• Provide brief behavioral health interventions using evidence-based techniques.
• Track treatment response and monitor changes in symptoms.
• Maintain therapeutic relationships via telephone and tele-video.
• Collaborate with Psychiatric Consultant on medication management.
• Deliver weekly patient care plan reviews with care team.
• Identify patients needing more intensive mental health care.
