A Hiring Company seeks a Behavioral Health Case Manager to deliver evidence-based psychological and behavioral health services in post-acute and long-term care settings. The role involves direct clinical care, assessments, and collaboration with interdisciplinary teams to support patient well-being and reduce hospitalizations.

An independently licensed clinician able to provide psychotherapy, crisis response, and care coordination is required, with adherence to Florida regulations.

Qualifications

• Must hold an active Florida independent license (LCSW or LMHC)
• Minimum of two years of clinical behavioral health experience
• Experience with geriatric or post-acute populations is preferred

Responsibilities

• Provide psychotherapy using evidence-based interventions for post-acute and long-term care residents.
• Deliver behavioral interventions to address mood, cognitive, and behavioral symptoms.
• Conduct psychological and behavioral assessments to support diagnosis and care planning.
• Collaborate with physicians, nursing, therapy, and social services to support integrated care.
• Complete documentation in the EHR adhering to regulatory standards and payer requirements.
• Provide crisis assessment and intervention and follow risk management protocols.
