Job summary

Wellbox is seeking a Collaborative Care Manager (CBCM) in North Carolina to join a multi-provider primary care team. You will oversee a caseload of about 75-80 patients, coordinating with PCPs and psychiatric consultants to develop active care plans and deliver brief behavioral health interventions via telehealth.

The role requires an MSW and an active NC LCSW license, with 2-3 in-office days per week in Huntersville.

Qualifications

• Master of Social Work (MSW) degree from an accredited program.
• Current, unrestricted Licensed Clinical Social Worker (LCSW) license in the state of North Carolina (required).

Responsibilities

• Support the mental and physical health care of patients across an assigned caseload of approximately 75-80 patients.
• Assess, plan, implement, monitor, and evaluate options and services to meet the psychosocial health needs of patients.
• Provide brief behavioral health interventions using evidence-based techniques such as behavioral activation, problem-solving treatment, motivational interviewing, or other treatments as appropriate.
• Systematically track treatment response and monitor patients for changes in clinical symptoms.
• Build and maintain continuous therapeutic relationships via telephone and tele-video platforms.
• Engage patients outside of normal clinic hours as necessary to fulfill CBCM duties.
• Collaborate with the Psychiatric Consultant to support psychotropic medication management as prescribed by medical providers, focusing on treatment adherence, side effect monitoring, and treatment effectiveness.
• Deliver weekly patient care plan reviews with the psychiatric care team and collaborative care team members.
• Facilitate and support regular collaborative case reviews with medical providers by telephone or video.
• Utilize a shared technology platform to track all assigned patients and prioritize patients for case review.
• Identify and escape patients who may need more intensive or specialized mental health care, as appropriate.
• Respond to communications from team members, patients, and medical providers in a timely manner.
• Perform other related duties as assigned.

Position Overview:

The Collaborative Care team includes the patient’s Primary Care Provider (PCP), a Psychiatric Consultant, Clinical Behavioral Care Manager (CBCM), and, when available, other mental health providers in the primary care clinic. The CBCM will carry a caseload of approximately 75-80 patients and will provide assessment and care management services, including administration of validated rating scales, technology-enabled psychosocial interventions, and behavioral health care planning for psychiatric health concerns. Responsibilities include revision of care plans for patients not progressing or whose status changes, maintenance of the online member registry, and ongoing collaboration with partner practitioners and the psychiatric care team.

Work Location:

This is a hybrid position requiring 2-3 days per week in office at 13620 Reese Blvd E, Suite 100, Huntersville, NC 28078.
