Responsibilities

• Collaborate with the assigned caseworker to identify and secure foster or kinship care homes.
• Support kinship and foster care families to maintain placement and pursue reunification goals.
• Provide ongoing support, behavioral coaching, and required training for providers.
• Maintain a caseload of foster care and kinship families (certified and non-certified).
• Engage with Adams County CAFS offices to coordinate services.
• Document contacts in TRAILS monthly and relay significant information to the treatment team.
• Assist providers with referrals including CCAP for childcare.
• Lead family engagement and reunification efforts with providers.
