Job summary

Child & Family is seeking a Case Manager in Rhode Island to work with EOHHS/DHS/BHDDH to provide home-based case management under the Conflict Free Case Management Model. You will perform in-home visits, monthly outreach, and develop person-centered plans within Medicaid Long Term Supports and Services guidelines.

Hybrid role with 20% remote work, requiring statewide travel and strong coordination with families, providers, and authorities.

Qualifications

Associate degree in social work, psychology, or related field required; bachelor’s preferred.
Minimum 1 year of case management experience; elderly or disabled population preferred.
Able to navigate an electronic medical records system.
Strong organizational, critical thinking, and time-management skills.
Must have valid driver's license, auto insurance, and car registration.
Willingness to travel statewide.
Bilingual in Spanish a plus.

Responsibilities

Manage cases assigned by program leadership or designee, with comprehensive documentation.
Conduct monthly participant outreach, including home visits.
Create Person-Centered Plans and support chosen by the participant.
Address participant concerns in a timely manner.
Provide person-centered, quality case management services.
Conduct in-home re-assessments to determine care needs.
Follow-up after plan implementation to reassess needs.
Collaborate with family/caregivers, service providers, and local authorities.
Complete all paperwork promptly and maintain accurate records.
Advocate for services to meet participant needs.
