Job summary

UNC-Chapel Hill in North Carolina seeks a part-time Family Navigator to support the Whole Brain Health Clinic (WBHC) at Chapel Hill. This hybrid role assists families, participates in clinic hours (Wed 7:45am–5pm) and coordinates with UNC and NC care navigators to identify goals and next steps.

The ideal candidate has an Associate’s degree in a related Human Services field and two years of relevant experience, with strong English and Spanish communication, and proficiency in Word, Teams and

Qualifications

• Associate’s degree in a related Human Services discipline with two years of related experience.
• Fluent English and Spanish communication skills are preferred.
• Proficiency in Microsoft Word, Teams and Excel is expected.

Responsibilities

• Support the WBHC clinic, in-person and via telehealth, assisting families.
• Attend clinic and interact with patients, describing the role of Family Navigator.
• Coordinate releases of information, consent processes, and goals with care teams.
