Job summary

The University of North Carolina at Chapel Hill in Carrboro, NC is seeking a Human Services Practitioner - Advanced to join the CIND program. This part-time permanent role (25 hours/week) supports the Whole Brain Health Clinic, providing navigation and family support within an interdisciplinary team.

The position requires an associate degree and two years' related experience, with bilingual English and Spanish communication preferred.

Qualifications

• Associate’s degree in a related Human Services discipline and two years of experience related to the area of assignment.
• All degrees must be received from appropriately accredited institutions.
• Experience working with epilepsy and IDD populations is a plus and bilingual communication is preferred.

Responsibilities

• Support the WBHC in-person clinic operations.
• Interact with patients and families to describe the Family Navigator role and set goals.
• Participate in care team meetings and coordinate with navigators for resources.
• Assist with telehealth consults and outreach to families.
