Job summary

Mosaic invites a compassionate professional to join as Mosaic at Home Coordinator/Case Manager in a hybrid role serving Pottawattamie County. You will connect with individuals, families, host home providers, and direct care workers to ensure personalized support and a strong, shared voice for those served.

You will develop Individual Support Plans, oversee service delivery and compliance, and provide on-call support to providers.

Qualifications

• A Bachelor’s degree in human services or related field is preferred, with at least two years of related work experience.

Responsibilities

• BuildEpicConnections: develop relationships with individuals, families, host homes and direct care workers.
• Be a Dream Champion: help individuals articulate goals and ensure their voices are heard and respected.
• Relationship Builder: monitor and optimize matches between individuals and home providers.
• Architect Personalized Plans: collaborate to create Individual Support Plans meeting needs and regulations.
• Keep it Real in the Field: oversee service delivery and conduct regular home visits for compliance.
• Be the Safety Net: provide on-call support to providers and direct care workers.
