Mosaic is seeking a compassionate Mosaic at Home Coordinator/Case Manager to empower individuals with intellectual and developmental disabilities in Pottawattamie County. Hybrid work model; salaried position with on-call coverage and travel for home visits (agency vehicle or mileage reimbursement available).
You will build strong relationships with individuals, families, host home providers, and direct care workers; develop Individual Support Plans; monitor service delivery and ensure regulatory
Qualifications
- Bachelor's degree in human services or related field preferred.
- Minimum of two years of related work experience.
Responsabilités
- Build rock-solid relationships with individuals served, families, host home providers, and direct care workers.
- Help individuals express goals and needs and ensure their voices are respected.
- Monitor relationships between individuals and home providers to ensure matching is positive and supportive.
- Collaborate with service planning team to create Individual Support Plans.
- Oversee service delivery and contract compliance by visiting homes and service environments.
- Provide on-call support to providers and direct care workers as needed.
