Mosaic is seeking a compassionate Mosaic at Home Coordinator/Case Manager to empower individuals with intellectual and developmental disabilities in Pottawattamie County. This salaried, hybrid role includes on-call support, home visits, and travel up to 2 hours with agency vehicle or mileage reimbursement.
You’ll build strong relationships, develop individualized support plans, monitor service delivery, and advocate for the people served in collaboration with families and providers.
Competenze
- Bachelor's degree in human services or related field preferred with minimum two years of related work experience.
- Experience with intellectual disabilities is a plus
Mansioni
- Build epic connections with individuals, families, host home providers and direct care workers.
- Be a dream champion: help individuals express goals and ensure their voices are heard.
- Monitor relationships between individuals and home providers to ensure a positive match.
- Architect personalized plans by collaborating with service planning to create Individual Support Plans.
- Oversee service delivery and contract compliance by visiting homes and service environments.
- Provide on-call support to providers and direct care workers as needed.
