Mosaic is seeking a dedicated Mosaic at Home Coordinator/Case Manager in a hybrid role serving Pottawattamie County. You will build strong relationships with individuals, families, host homes, and direct care staff while assessing needs and ensuring compliant service delivery.

Qualificações

• Bachelor’s degree in human services or related field with at least two years of related experience.
• Experience working with people with intellectual disabilities is a plus.
• Valid driver's license in good standing for home visits.
• Strong communication, relationship-building, problem-solving, and computer skills.
• Knowledge of HCBS regulations is a plus.

Responsabilidades

• Build epic connections with individuals, families, host home providers, and direct care workers; provide exceptional customer service.
• Help individuals express goals and ensure their voices are heard and respected.
• Monitor relationships between individuals and home providers to ensure a positive match.
• Collaborate with service planning team to create Individual Support Plans that meet wishes and regulatory requirements.
• Oversee service delivery and contract compliance; conduct regular home and service environment visits.
• Provide on-call support to providers and direct care workers when needed.
