Job summary

Mosaic is hiring a Targeted Case Manager to support people with developmental disabilities. The role combines community engagement with documentation and care planning to enhance independence and integration of individuals served.

You will work with the person and support network to identify and obtain paid services and natural supports, within the person’s lifestyle and needs. Hybrid schedule, with field and office presence.

Qualifications

• Six months of experience in human services.
• Bachelor's degree or additional full-time experience in developmental disabilities services.
• Ability to navigate and connect individuals with community services.

Responsibilities

• Develop, update and review the Person-Centered Support plan with the person and their support network.
• Monitor quality of services delivered to individuals served.
• Document progress and maintain records for persons served.
