Job summary

The General Hospital Corporation is seeking a Nurse Case Manager to support Home Base COMBHAT, coordinating care for Veterans and service members. The role involves liaison among providers, patients, families, and referral sources across the Mass General Brigham network.

Onsite Thursdays and Fridays with additional onsite support as needed, you will ensure care coordination, plan discharge activities, and identify local resources to complete treatment goals in collaboration with the Brain Health

Qualifications

• Associate's Degree Nursing required or Bachelor's Degree Nursing preferred.
• Experience in case management, utilization review, or discharge planning desired.
• Knowledge of healthcare and managed care preferred.

Responsibilities

• Conduct client assessments and planning to establish care goals.
• Modify patient treatment plans as indicated by responses and conditions.
• Prepare patient/family for discharge and coordinate with care teams.
• Communicate with third-party payers to obtain required authorization for reimbursement.
• Review cases with medical directors to discuss challenging cases.
• Maintain accurate, detailed reports and records of care.
• Develop and coordinate community service plans with patients and families.
