Job summary

Mass General Brigham Home Care is hiring registered nurses to provide skilled home care, continue ongoing patient assessments, and manage care plans in patients' homes across the MGB geographic area.

The position offers a hybrid work model with a 36-hour schedule (Tuesday–Friday, 9-hour days) and requires strong communication, collaboration with an interdisciplinary team, and travel within the service area.

Qualifications

• Graduate of an approved School of Nursing
• Current Massachusetts Registered Nurse license
• At least one year of acute care nursing experience (home care experience preferred)
• Comfort working with various computer systems; handheld clinical devices preferred
• Strong interpersonal and customer service skills with diverse populations
• Excellent communication skills—written, verbal, and electronic
• Strong organization and prioritization in a dynamic environment
• Ability to travel within the MGB Healthcare at Home geographic area

Responsibilities

• Complete patient assessments and admissions
• Develop and manage individualized care plans
• Provide skilled nursing care, education, and treatment
• Coordinate care with patients, families, and providers
• Document care and review charts
• Collaborate with the interdisciplinary team
• Deliver high-quality, patient-centered care
