Job summary

BrightonOne in the Boston area seeks an RN Case Manager to provide telephonic case management for US Family Health Plan members with chronic, complex, or high-risk conditions. This exempt, full-time role emphasizes coordinating care across providers and community resources to improve outcomes.

The position requires an ADN (BSN preferred), 2+ years nursing experience, 1+ year case management, MA RN license, CCM or CBHCM certification within 3 years, and the ability to work in a hybrid setting

Qualifications

• Associate’s degree in Nursing (ADN) required; BSN preferred.
• 2+ years of clinical nursing experience.
• 1+ year of case management experience, preferably in managed care.
• Massachusetts RN license active and unrestricted; CCM/CBHCM certification within 3 years.

Responsibilities

• Identify and enroll eligible members in case management.
• Conduct comprehensive assessments across clinical, behavioral, and psychosocial domains.
• Develop and update individualized care plans with measurable goals.
• Coordinate care across providers and settings for improved outcomes.
• Educate members and caregivers on conditions, plans, and benefits.
