Advocate Health seeks a Nurse Practitioner or Physician Assistant to join the Heart Failure Service at Advocate Lutheran General Hospital in Park Ridge, IL. The role splits 50% outpatient and 50% inpatient, delivering continuity of care across the heart failure spectrum.

You will work with a multidisciplinary team to optimize outcomes, conduct evaluations, titrate medications, and coordinate transitions from hospital to outpatient follow-up.

Qualifications

• Active RN license in state of practice, APRN/APRN-FPA/APNP license.
• Active national board certification in area of practice.
• Active DEA registration prior to hire.
• Illinois Controlled Substance License prior to hire (if Illinois practice).
• BLS and/or ACLS, PALS, NRP as required by clinical practice.
• Master’s Degree in Nursing.

Responsabilités

• Outpatient Heart Failure Clinic (50%): manage recently discharged patients, provide evaluation, education and monitoring.
• Inpatient Heart Failure Service (50%): partner with physicians, manage hospitalized patients and care transitions.
• Coordinate daily inpatient care, treatment planning, and discharge planning with the team.
