Job summary

Advocate Lutheran General Hospital in Park Ridge, IL seeks an Advanced Practice Provider (NP/PA-C) to join the Heart Failure Service. This role blends outpatient management and inpatient rounds, collaborating with a multidisciplinary cardiovascular team to improve outcomes.

The position emphasizes continuity of care from hospitalization through outpatient follow-up, with four 10-hour shifts and one Saturday every 8 weeks, no overnight duties.

Qualifications

• Active RN and APRN licenses in state(s) of practice.
• Active national board certification in area of practice.
• Active DEA registration prior to hire.
• Illinois CS license prior to hire (if Illinois practice).
• BLS/ACLS/PALS/NRP as required by practice.

Responsibilities

• Assess, diagnose and manage treatment plans for heart failure patients.
• Order, perform and interpret diagnostic studies and therapies.
• Coordinate inpatient and outpatient care with the cardiovascular team.
• Document accurately in the EMR and ensure compliant coding.
• Serve as liaison among patients, families, and care teams.
• Participate in quality, safety and education initiatives.
