Job summary

Advocate Aurora Health in Park Ridge, IL seeks a dedicated Nurse Practitioner or Physician Assistant to join the Heart Failure Service. The position offers an advanced practice pathway, with four 10-hour shifts weekly, a rotating Saturday rounding schedule every eight weeks, and no overnight duties.

Qualifications

• NP or PA-C with heart failure care experience.
• Ability to work in 50% outpatient and 50% inpatient settings.
• Strong collaboration with a multidisciplinary cardiovascular team.

Responsibilities

• Assess, diagnose, and manage heart failure patients across care settings.
• Coordinate care across inpatient rounds and outpatient visits with physicians, nurses, and pharmacists.
• Educate patients and families; ensure smooth transitions of care and prevent readmissions.
• Document accurately in the EMR and support care planning and discharge planning.
