Job summary

Loyola Medicine, a member of Trinity Health, is seeking an Advanced Practice Provider to join the Advanced Heart Failure Program in the Chicago area. The role blends inpatient and outpatient duties with focus on advanced heart failure, VAD support, and heart transplants.

You will work with physicians and a multidisciplinary team to coordinate rounds, care plans, and transplant evaluations, while expanding ambulatory services and improving access to specialized cardiovascular care.

Qualifications

• Current Registered Nurse License - State of Illinois (RN).
• Nursing Practitioner License - State of Illinois (APN).
• Physician Assistant License - State of Illinois (PA).
• ACLS certification.
• Basic Life Support (BLS) certification.

Responsibilities

• Provide comprehensive inpatient care for advanced heart failure, VAD, and transplant patients, including assessment and admissions.
• Coordinate daily rounds, documentation, and multidisciplinary care planning with physicians.
• Manage transfers, discharges, and transitions of care across teams.
• Assist in evaluation and management of transplant candidates and those awaiting advanced therapies.
• Help expand ambulatory/outpatient services to improve access to care.
• Contribute to initiatives to reduce readmissions and improve care coordination.
