Job summary

Centene is seeking a Field Care Coordinator based in Allegheny County to support member engagement through education, community resources, and collaboration with providers to close care gaps.

The role includes home visits and community assessments, documenting interactions, and coordinating care plans with internal teams to improve member health outcomes while meeting state and federal requirements.

Qualifications

• Bachelor's degree in Social Work, Public Health, Nursing, or related field.
• 2+ years in community health or healthcare quality.
• Experience working with health plan members and navigating community resources.
• Familiarity with Medicaid / Medicare programs and quality measures (e.g., HEDIS).
• Strong communication and interpersonal skills.

Responsibilities

• Perform home visits or community-based assessments to identify social determinants of health and care gaps.
• Educate members on preventive care and available community resources.
• Assist with scheduling appointments and navigating benefits or entitlements.
• Collaborate with providers and internal teams to coordinate care and improve outcomes.
• Document interactions and track care gap closures in systems.
