Job summary

You will educate on preventive care, assess social determinants of health, and coordinate with providers to close care gaps. Flexibility and strong communication are essential to support diverse member needs.

Qualifications

• Bachelor’s degree or equivalent experience required.
• 2+ years in community health or healthcare quality.
• Experience with health plan members and navigating community resources.
• Familiarity with Medicaid/Medicare and quality measures (e.g., HEDIS).
• Strong communication and interpersonal skills.

Responsibilities

• Outreach to community members to identify care gaps and connect to services.
• Perform home visits or community assessments to evaluate needs.
• Advocate for members; assist with appointments, plans, and benefits.
• Collaborate with providers to share quality data and support improvements.
• Educate on preventive care and available community resources.
• Document interactions, care gaps, and referrals in systems.
• Coordinate with internal departments to improve outcomes.
• Monitor outreach effectiveness and care-gap closure metrics.
• Maintain compliance with regulations and policies.
• Participate in campaigns to improve member engagement.
• Build relationships with local organizations and stakeholders.
• Performs other duties as assigned.
