Job summary

MedZed is hiring a Housing Navigator II to provide in-field housing support and case management for Medicaid members in the San Diego area. This full-time role requires travel to members' homes and community settings, with a hybrid/remote work option.

You will assess needs, create service plans, connect members to resources, and document activities in Salesforce Health Cloud, while upholding Housing First and trauma-informed principles.

Qualifications

• High School Diploma or equivalent.
• 1+ years of experience in housing services, case management, homelessness services, or social services.
• Knowledge of community resources.
• Ability to work in field-based settings and travel locally.
• Strong documentation and compliance skills.
• Ability to manage complex psychosocial and medical needs.
• Willingness to conduct outreach via phone and door knocks to enroll new members.
• Be in good standing with no active corrective action.

Responsibilities

• Manage a housing caseload up to 55+ members.
• Conduct intake and develop individualized service/housing plans.
• Assist with housing searches, advocate with landlords, and conduct home visits.
• Accompany members to housing appointments.
• Perform calls, field visits, and door knocks to enroll new members.
• Build and maintain a member panel through outreach.
• Coordinate referrals to health plans, providers, and community resources.
• Document timely, accurate case notes in Salesforce Health Cloud and portals.
• Participate in interdisciplinary case conferences and team huddles.
• Escalate high-risk member needs appropriately.
• Maintain compliance with CalAIM, DHCS, and health plan requirements.
• Meet outreach and documentation timeliness standards.
• Attend required training and professional development.
• Support audit readiness and data tracking activities.
