Job summary

Heluna Health is recruiting for an MCW II (FFA Coordinator) to manage full-time referral processing for Flexible Financial Assistance for People Experiencing Homelessness (FFA for PEH) in Los Angeles. You will handle referrals, eligibility, documentation, and payments, coordinating with Brilliant Corners and partner agencies.

The role works with HSH leadership and the Homelessness Prevention Unit, using HMIS and SalesForce to track outcomes, with potential hybrid work options and a focus on

Qualifications

• One year of experience as a Medical Case Worker I OR a Bachelor's degree with one year of professional casework interviewing, counseling, and assisting patients/clients.
• Experience coordinating with external partners and vendors to support client needs.
• Knowledge of data systems used in homeless services (e.g., HMIS, SalesForce).

Responsibilities

• Review FFA referrals submitted by partner agencies and screen for participant eligibility and priority.
• Verify referring party’s attempt to utilize other funding sources before approving requests.
• Review and approve financial assistance requests and ensure supporting documentation is provided.
• Maintain communication with referring agencies regarding eligibility, fulfillment timeframes, and pathways to other resources.
• Maintain strong communication with third party vendors and Brilliant Corners involved in payment processing.
• Assist with data reporting, analytics, and development of budgeting tools.
