Job summary

BoldAge PACE in Holly Hill, FL is seeking an Enrollment Specialist to guide prospective participants and families through the enrollment process, from intake to program enrollment, converting referrals into enrollments. The role acts as the primary contact, building trusted relationships and explaining benefits and requirements of the PACE program.

You will work with Outreach, Benefits, and Interdisciplinary teams, coordinate enrollment activities, manage timelines, and travel within the service

Qualifications

• High school diploma or equivalent required.
• Bachelor’s degree preferred.
• 1-3 years of customer-facing experience required; healthcare intake, call center, admissions or inside sales preferred.
• Experience managing high-volume inbound/outbound communications (phone, text, email).
• Experience with CRM systems and maintaining timely documentation.
• Knowledge of PACE, Medicaid, Medicare or healthcare programs preferred.
• Proficiency in Microsoft Office suite.
• Strong written, verbal, and interpersonal communication skills.
• Experience with older adults, caregivers, or vulnerable populations preferred.

Responsibilities

• Owns prospects after handoff from Enrollment Advisor when ready to enroll or needs in-person support.
• Guide qualified participants through post-intake to completed enrollment, conducting consultative conversations.
• Clearly present program services, eligibility, and enrollment expectations to participants, caregivers, and family.
• Convert referrals into enrollments by guiding decision-making and in-person experiences.
• Coordinate enrollment activities with Benefits Specialists, Level of Care Assessors, and Interdisciplinary Team.
• Manage enrollment timelines to ensure timely progression and resolve delays.
• Travel within service area to meet prospects in homes or facilities.
• Collect all enrollment documentation and medical records, coordinating with providers.
• Conduct the Enrollment Conference and finalize enrollment per CMS regulations and Enrollment Agreement.
• Support Medicaid eligibility navigation in collaboration with Benefits Specialists and state agencies.
• Represent participant needs during discussions with the interdisciplinary team.
• Maintain communication with prospects and partners throughout enrollment.
• Document enrollment activities in CRM and EMR systems.
• Assist with initial transition into PACE during first three months.
