Job summary

BoldAge PACE is seeking an Enrollment Specialist to guide prospective participants and families through the enrollment process, converting qualified referrals into program enrollments while representing our mission of putting people first.

You will collaborate with Outreach, Benefits, and clinical teams, coordinate enrollment activities, manage documents, and ensure CMS requirements are met, with travel within the service area and a hybrid work arrangement.

Qualifications

• Minimum 1-3 years of experience in a customer-facing role.
• Experience in healthcare intake, call center operations, patient access, admissions, or inside sales preferred.
• Experience with high-volume inbound/outbound communications (phone, text, email).
• Proficient with CRM systems and documentation.
• Knowledge of PACE, Medicaid, Medicare or health care programs preferred.

Responsibilities

• Guide prospects through enrollment and convert referrals into enrollments.
• Coordinate enrollment activities with internal partners and teams.
• Maintain CRM and EMR documentation.
• Travel within service area to meet prospects.
• Support enrollment through CMS enrollment requirements and materials creation.
• Communicate with participants and families about program benefits.
