Job summary

Delta Dental of Michigan, Ohio, and Indiana seeks a PCDH Coordinator and Tribal Liaison to administer the Patient Centered Dental Home assignment and related activities, including system updates, provider and member requests, reporting, and stakeholder communication.

They will serve as the primary organizational liaison to Tribal Nations and health programs, collaborating with internal and external partners to advance dental home initiatives and culturally responsive outreach.

Qualifications

• Bachelor’s degree in a relevant field with three years of experience in patient care coordination, healthcare program administration, community outreach, population health, or public health, and Tribal background or experience with Tribal Nations.
• Strong cultural competency skills with the ability to develop culturally responsive programs and communications.
• Excellent communication, presentation, project management, and stakeholder engagement skills.

Responsibilities

• Supports DDMI and Contractor priorities by monitoring utilization and promoting high-value preventive services.
• Implements Dental Home Strategy outreach, coaching, education, and engagement activities for members and providers.
• Collaborates with the project management team to update and enhance Dental Home workflows.
• Reviews reports and utilization trends to identify opportunities and barriers to care utilization.
• Promotes access to preventive services through trusted primary care dentists to reduce avoidable restorative care needs.
• Facilitates communication among members, providers, MCOs, case managers, and community partners.
