Qualifications

• Bachelor-level degree in dental hygiene, public health, health administration, or related field.
• Three years of experience in dental hygiene, public health, population health, outreach, care coordination, community health, or related healthcare programs.
• Two years of leadership experience; professional certification such as CPHQ or HCQM preferred.

Responsibilities

• Plan, develop, implement, and evaluate population health and quality strategies.
• Analyze quality and utilization data to identify trends and gaps.
• Develop outreach plans for Medicaid, Medicare, and government programs.
• Oversee care coordination and staff performance and development.
• Provide SME on population health, health equity, and quality measures.
